§ 22-52. Application procedure.

Application for a building permit shall be made to the city building department on a form to be prescribed by the city. At a minimum, the applicant for a building permit shall state the following:

(1)

The name of the owner of the property.

(2)

The street address of the property.

(3)

The address of the owner if different from the street address of the property.

(4)

The nature of the work to be performed.

(5)

The value of the improvement.

(6)

The expected beginning date.

(7)

A drawing of the planned improvement.

(8)

Proof of public liability and property damage insurance coverage as required by F.S. § 489.115 and worker's compensation insurance coverage as required by F.S. § 489.114.
